Observation

Provider was not meeting group size and staff-to-child ratio at time of incident as there were 26 children on the play yard and two staff members. Licensor find it is more likely than not that staff were not actively supervising outdoor play at time of injury. Director said staff were not positioned to supervise all areas accessible to children and were not aware of what all children were doing at the time of incident.
